Which command do I use for my version?

On Minecraft 1.20.5 and newer use the "Component" command (it uses readable pattern and colour names). On 1.13–1.20.4 use the "Legacy NBT" command. Both are generated for you.

How do I turn a banner into a shield?

Craft the finished banner with a shield in a crafting table, or run the same /give on a shield item — the banner_patterns component and BlockEntityTag both work on shields too.

Why are creeper, skull and Mojang patterns missing?

Those "picture" patterns need a special banner pattern item to apply and only ever produce one fixed shape, so they are left out. Every geometric pattern you can make on a loom is here.

How many layers can a banner have?

A loom limits you to 6, but the /give command accepts up to 16 layers stacked on top of the base colour, so that is the limit here.
